Sex Workers In Pakistan Give Recommendations On HIV Prevention To Health Officials
Although the recorded HIV prevalence in Pakistan is relatively low, health officials are concerned that a concentrated epidemic of the virus among injection drug users could carry over to commercial sex workers and other high risk groups in the country, IRIN/PlusNews reports. To address the issue, the National AIDS Control Program and the United Nations Population Fund recently held a meeting, called the National Consultation on HIV and Sex Work, in an effort to improve HIV prevention efforts targeted at sex workers by consulting with workers in the field.Sex workers at the meeting made various recommendations, including HIV testing, referrals and increased efforts to decrease stigma. The Ministry of Health reports that from 2006 to 2007, female sex workers were at a high risk of HIV in 12 cities across Pakistan. A survey of 4,639 female sex workers found that less than 25% reported condom use; 10% had a partner that had used injection drugs during the past six months; and that illiterate sex workers were less likely to use condoms than those with a higher level of education. A female sex worker at the meeting said, "It is very hard for us to convince [partners] to put on a condom, but I feel that a female condom would put us in a position where we can protect ourselves against HIV and sexually transmitted infections." She added that female condoms are not widely available. Legalizing sex work would make it easier for sex workers to protect their rights, another female sex worker at the conference said. She added that often, outreach workers face barriers from police forces. Daniel Baker, UNFPA"s country representative for Pakistan, said that sex workers should have greater involvement in creating and implementing HIV programs. He added, "The female sex workers have to be in there as managers, workers and leaders to benefit in the long run." Safdar Kamal Pasha with UNFPA agreed that the recommendations from sex workers are critical points to address in future programming. "The female sex workers agreed that there should be vocational training and the means for alternative work opportunities for those who want to move out of sex work, as well as those who are past their prime and do not find sustainability in sex work," Pasha said (IRIN/PlusNews, 5/14).

Health Information Technology Lobby Group Rallies Support For Certification Group; Critics Question Group's Ties
The Healthcare Information and Management Systems Society has asked HHS to give the Certification Commission for Healthcare Information Technology authorization to determine which electronic health records systems can receive funding from the economic stimulus package, the Washington Post reports. In a letter dated April 27 to HHS officials, HIMSS officials wrote, "To ensure continuity, recognize CCHIT as the certifying body" of EHRs.Some health care industry officials have raised issue with giving CCHIT the responsibility of certifying EHR products because of the commission"s associations with various IT and health care companies, the Post reports. CCHIT has ties with HIMSS, which played a role in its inception in 2004 and is now managed by Mark Leavitt, the former chief medical officer of HIMSS. In 2005, the commission received a three-year, $7.5 million contract from HHS.According the Post, Internal Revenue Service tax documents show that HIMSS technically paid Leavitt"s salary through 2008, which was reimbursed by CCHIT. However, Leavitt said he is accountable only to CCHIT"s board members and he "was not supervised by HIMSS." He said he expects CCHIT will be "the body or one of several certifying bodies that are recognized" by HHS in part because it already is tasked with certifying health IT products. According to Leavitt, some of the commission"s critics are IT vendors who have failed to meet CCHIT"s standards. The Post reports that the provision in the stimulus package that requires health care providers to demonstrate "meaningful use" of health IT has become an issue because federal officials, IT systems vendors, and physicians and patient advocates have not been able to reach a consensus on the definition of meaningful use. New Information About DNA Repair Mechanism Could Lead To Better Cancer Drugs

Researchers at Washington University School of Medicine in St. Louis have shed new light on a process that fixes breaks in the genetic material of the body"s cells. Their findings could lead to ways of enhancing chemotherapy drugs that destroy cancer cells by damaging their DNA. Using yeast cells, the scientists studied protein molecules that have an important role in homologous recombination, which is one way that cells repair breaks in the DNA double helix. The process in yeast is similar to that in humans and other organisms. Earlier research had established that a protein molecule named Srs2 regulates homologous recombination by counteracting the work of another protein, Rad51. Reporting in the July 10 issue of the journal Molecular Cell, the research team reveals the mechanism of how Srs2 removes Rad51 from DNA and thereby prevents it from making repairs to broken strands. "Our findings may make it possible to uncover ways to augment the effect of DNA-damaging agents that are used for cancer chemotherapy," says senior author Tom Ellenberger, D.V.M, Ph.D., the Raymond H. Wittcoff Professor and head of the Department of Biochemistry and Molecular Biophysics. "Many chemotherapeutic agents work by causing DNA damage in cancer cells, leading to their death, and tumors can become resistant to chemotherapy by using DNA repair mechanisms to keep the cells alive. Drugs that inhibit the DNA repair process could help increase the efficiency of chemotherapeutic agents." Ellenberger is also co-director of the Pharmacology Core at Siteman Cancer Center at Barnes-Jewish Hospital and Washington University. The facility aids in the development of anti-cancer agents. Srs2 is a helicase molecule a motor protein that"s able to walk or slide along a strand of DNA and remove other proteins from DNA or separate the two strands of the twisted double helix. For studies of Srs2, Ellenberger"s laboratory collaborated with Timothy Lohman, Ph.D., the Marvin A. Brennecke Professor of Biochemistry and Molecular Biophysics, a prominent expert in the biochemistry of motor proteins like Srs2. Rad51"s job in the cell is to promote the exchange of sequences between two related DNA molecules, which can be used to repair breaks in DNA where both strands of the double helix are compromised. As a DNA matchmaker, Rad51 forms long filaments on DNA. Srs2 can remove these to prevent unwanted exchanges of DNA sequences. Without Srs2, cells lose their ability to maintain the normal structure of chromosomes, and DNA sequences become shuffled. The biochemists found that Srs2 possesses a small arm that interacts with Rad51 and triggers a chemical reaction within the Rad51 protein causing it to fall off the DNA. "Scientists had assumed that as Srs2 moved along the DNA strand, it just pushed off everything in its path," says lead author Edwin Antony, Ph.D., a postdoctoral research associate in biochemistry and molecular biophysics. "This isn"t the case we showed that Srs2 has a specialized structure that allows it to interact specifically with Rad51." This finding shows how a motor protein like Srs2 can perform the specialized task of remodeling a protein-DNA complex without interference by other similar helicases, he adds. Because they now know more precisely the nature of this interaction between Srs2 and Rad51, the researchers can narrow their search for drugs that will block DNA repair by Rad51. This type of drug could make a lower dose of a DNA-damaging drug effective in treating cancer. The research team is now trying to identify the Srs2 homologue in human cells and will study its structure in combination with Rad51. That will allow a more rational approach to understanding how cells cope with DNA damage and how some tumors evade cancer therapeutics, they say. "In the long-term, my laboratory will look for drug-like molecules that influence this interaction," Ellenberger says. "We are using the Chemical Genetics Screening Center here at the University (http://htc.wustl.edu). It has vast libraries of molecules that may have the activity we want. Edwin"s work on Srs2 and Rad51 will allow us to develop an assay to screen for agents that augment or supersede Srs2"s interference with DNA repair."
